Summary


There are two parts to the improving elective services health target:

  • maintaining compliance with Elective Services Patient Flows Indicators (ESPIs)
  • delivering an agreed increase in the level of elective service patient discharges.
Elective Services Patient Flow Indicators (ESPIs)
For patients referred for elective (non-urgent) assessment or treatment there are some decision points about priority and access to treatment. The indicators (ESPIs) monitor how effectively DHBs manage the patients referred for elective assessment or treatment.

  • 07/08 Target Achievement
    Thirteen out of 21 DHBs were ESPI compliant every month of the year. Another six DHBs were compliant for nine months of the year.
  • Quarter Three 2007/08
    The way DHBs manage their elective systems has remained at a similar level to quarter two. Five DHBs have rated as outstanding by meeting the targets that they have set. These targets are better than the Ministry compliance threshold. Also, 10 DHBs have achieved the Ministry target.
  • Quarter Two 2007/08
    There has been a small improvement in the way DHBs manage their elective systems. Fifteen DHBs now have either an achieved or outstanding rating through meeting the targets that they have set. These targets are better than the Ministry compliance threshold. Northland, Taranaki, Nelson Marlborough and Otago DHBs have all received an “Outstanding” performer rating for both quarters.
  • Quarter One 2007/08
    Significant progress was made, with DHBs improving the way that their elective systems are managed. This improvement is reflected in the results, with two thirds of DHBs receiving achieved or outstanding ratings for the ESPI part of the Health Target.

Discharge volumes
DHBs have agreed a base (minimum) number of elective services to be delivered each year. Each DHB has also set a target to deliver an additional (extra) level of elective service discharges to their population. These services may be delivered by the local district hospital, or by a hospital outside the DHB region for specialty services, for example, cardiac surgery. The health target includes surgical service discharges, plus dental, cardiology and some outpatient services for example endoscopy.

  • 07/08 Target Achievement
    In 2007/08, an increase of
    11,864 elective discharges was achieved. This is an 8.7% increase against a target of 10%.
  • Quarter Three 2007/08
    Progress in this area has deteriorated slightly. Seven DHBs have delivered services in excess of their planned target. Overall, at the end of March 2008 a 6 percent increase has been achieved, representing an additional 6055 discharges. A 10 percent increase (9837 discharges) was planned. Many DHBs have timed the delivery of additional service in the third and fourth quarters of 2007/08. Delays in coding some discharges and capacity constraints in some DHBs continue to affect the results.
  • Quarter Two 2007/08
    At the end of December 2007, a six per cent increase in discharge volumes was achieved; this represents an additional 3738 patient discharges. A nine per cent increase was planned. Progress in this area has improved for the second quarter with nine DHBs delivering services in excess of their planned target. Auckland, Wairarapa and South Canterbury DHBs have all delivered more than 20 per cent additional discharges to their planned targets. Delays in reporting discharges and capacity constraints in some DHBs continue to affect the results in this target area.
  • Quarter One 2007/08
    Progress in this area was slow, with the amount of elective services delivered being lower than the amount planned for the first quarter. Two thirds of the DHBs have not achieved delivery of their planned target volumes. There are a range of contributing factors to this result, including variability in first quarter volume delivery, delays in DHB completion of reporting and capacity constraints

National trend in elective services increase

Graph of the national trend in elective services discharges.
This graph presents a time series for in-patient surgical elective services (including dental) and cardiology - it is a subset of the national target and represents 89% of the total Health Target. It excludes outpatient procedures such as endoscopy, lithotripsy and minor general surgical procedures that will not be included in future years’ Health Targets.
